Abstract

Further development of total ankle replacements (TAR) has led to the latest 4th generation prostheses. The CADENCE ankle prosthesis is a primary replacement designed to match native anatomy and function as close as possible through several key design features: (1) anatomically shaped, side specific, tibial component with unique syndesmotic “cutout” to provide 3 cortices support and reduce fibular impingement, (2) anatomically shaped, side specific, bi-condylar talus with central trochlear groove for enhanced stability, (3) minimal bone resection talar component with specially targeted cut guides to avoid talar blood supply, (4) enhanced sagittal plane ankle stabilization with available anterior and posterior biased poly inserts, and (5) efficient, streamlined instrumentation designed for the ambulatory surgery environment.

The detailed surgical technique is reviewed, with important surgical insights and technique pearls highlighted and shared by the authors. Early 2-year results are discussed as well as future design enhancements.
