Abstract

Transition from pediatric to adult care for adolescents and young adults with and without chronic illness has been a growing area of research for many years. However, most research has focused on single disease experiences with limited outcomes and most often at a one institution. This review will describe common qualitative and quantitative research outcomes used in transition research and outline an agenda for future research utilizing the quadruple aim framework, including improved patient experiences, better health outcomes, lower cost of care, and improved provider experience.
