Abstract
Speaking appears simple. It is only when it is impaired or absent that we are conscious of its importance. The rehabilitation of breathing and voice and thus speech is one of the four topic areas in the F.O.T.T. approach. F.O.T.T. is not limited to the face, mouth or the swallowing process. The functions of breathing, swallowing and voice are interconnected and influence each other. They are reliant on posture, movement and sensation. For each system to work efficiently they need to coordinate with each other. When working therapeutically, the complex interaction and coordination required between the sensorimotor system, respiration and swallowing to produce voice and articulate words intelligibly is apparent. The F.O.T.T. approach considers how posture, muscle tone, sensation and movement of the whole body influence facial and oral movement, breathing, voice and speech. This chapter focuses on typical issues faced by severely affected patients and their treatment.
