Failure of Erythrocyte Production

Abstract

Disorders resulting in failure of erythrocyte production can be broadly divided into (a) decreased rate of erythrocyte production in the bone marrow and (b) defects in erythrocyte maturation and subsequent ineffective erythropoiesis.

Decreased rate of erythrocyte production indicates a defect in production due to primary bone marrow disease or a defect in the stimuli that causes erythrocyte production in the bone marrow. Accordingly, they can be further subdivided into (a) intrinsic bone marrow failure that includes aplastic anemia, congenital erythrocyte production disorders like Diamond-Blackfan syndrome, transient erythroblastopenia of childhood and Fanconi’s anemia, and (b) defects in stimuli for the bone marrow, i.e., impaired erythropoietin production.

Defects in erythrocyte maturation and ineffective erythropoiesis mostly result from the lack of sufficient nutrients in the body to produce erythrocytes. These can be subdivided into (a) abnormalities in cytoplasmic maturation, (b) abnormalities in nuclear maturation, and (c) congenital dyserythropoietic anemias.

This chapter focuses on some of the more common disorders in each of these groups.
