Screening Tools for Evaluating the Bleeding Adolescent

Abstract

Adolescents frequently present to the clinician with a concern for “abnormal” bleeding including bruising, epistaxis, heavy menstrual bleeding, and surgical bleeding. The subjective experience of bleeding as excessive or prolonged can be difficult to evaluate. Bleeding assessment tools comprised of questionnaires and pictorial descriptions with a scoring system have evolved over the last 40 years as an objective means of quantifying bleeding. A validated bleeding assessment tool can thus assist the provider in differentiating “normal” vs “abnormal” bleeding. As there are several bleeding tools and instruments available to clinicians, when encountering a patient with a bleeding complaint, it is important to understand which tools may yield the best diagnostic information for that individual. This chapter will illustrate and compare various bleeding assessment tools as well as discuss the history, development, and future application of these instruments. Use of the appropriate bleeding assessment tool can greatly enhance the diagnosis and management of bleeding disorders and can reduce unnecessary and costly testing.
