Abstract

Ampullary tumors are those arising directly from the structures of the ampullary complex distal to the confluence of the bile duct and pancreatic duct and represent roughly ~7% of periampullary neoplasms. Ampullary carcinoma carries a more favorable prognosis than other pancreaticobiliary malignancies. Surgical resection remains the mainstay of therapy for resectable ampullary carcinoma. Curative-intent resection is possible in ~50% of patients presenting with ampullary cancer compared with ~10% for patients with pancreatic cancer. Limited evidence is available to guide the use of adjuvant therapies due to the relative rarity of this histologic subtype. Recommendations for adjuvant therapy are largely extrapolated from studies for other pancreaticobiliary malignancies. Multidisciplinary management in experienced centers is advised for these rare and complex tumors.
