Abstract

The screening of secondary hypertension should include the detection of vascular disease. Aortic coarctation is one of the most common causes in children but is infrequent in adults. Few patients with aortic coarctation remain undetected until adult life and by then often have severe hypertension, hypertension-mediated organ damage and an extensive collateral circulation below the coarctation. Takayasu arteritis, a rare large vessel vasculitis of unknown etiopathogenesis, is often associated with hypertension. The aims of this review are to address the issues of aortic coarctation and Takayasu arteritis in patients with hypertension and the possible mechanisms explaining elevated blood pressure. This review does not address renal artery disease, such as atherosclerotic stenosis or fibromuscular dysplasia since these are described in other chapters of this book.
