Abstract

Treatment of acute ischemic stroke has been revolutionized in the recent years with a series of successful clinical trials. Advanced neuroimaging plays a central role in selecting patients for endovascular intervention or thrombolytic therapy. For patients within the early-treatment window (0–6 hours), NCCT and CTA can usually provide enough information. For late-window (6–24 hours) patients, either CT perfusion or MRI DWI with or without MR perfusion is recommended to estimate the volume of infarction and/or penumbra and select candidates for thrombectomy. This chapter will discuss the application of CT- and MRI-based techniques in acute stroke imaging, with emphasis on the pearls and pitfalls of CT perfusion.
