Rapid Recovery in Total Joint Arthroplasty

Abstract

In the changing world of total joint arthroplasty (TJA), fewer patients discharge to skilled nursing facilities (SNFs) than in the past. More efficient perioperative assessment and optimization, improved anesthesia, multimodal pain management, and an emphasis on early mobility have made discharge to SNF unnecessary for the majority of patients undergoing TJA. In addition, discharge to SNF is associated with increased morbidity and cost, making discharge to SNF less favorable. Bundled payments are becoming more prevalent and are significantly impacted by discharge to SNF, making cost even more of a factor. However, certain patient populations need higher levels of care for a safe postsurgical recovery, and SNFs play an important role for these patients. Thus, good relationships with SNFs are critical in order to provide safe and cost-effective care for those patients requiring them.
