Abstract
This chapter presents a model for integrated psychosocial and pharmacological treatment for self-injurious behavior, incorporating behavioral and other psychosocial components with rational pharmacological elements and considering possibilities for stratified intervention. We review the evidence base relevant to behavioral interventions for emergent, operant, and stereotyped self-injury, and behavioral and pharmacological treatment of habitual/impulsive and compulsive self-injury, considering evidence for effectiveness of and possible adverse events associated with pharmacological and behavioral interventions. Specifically, we review in detail evidence related to use of noncontingent reinforcement, functional communication training, progressive exposure with reinforcement, and “second-generation” or atypical antipsychotic medications. The chapter describes clinically relevant methods for wide-range assessment of treatment impact. Issues in the long-term maintenance of behavioral components of interventions are discussed, as are methods for maintaining treatment integrity and the robustness of these interventions if intervention fidelity is compromised over time. The issue of “relapse” following successful intervention, and appropriate clinical intervention in the face of relapse, is also considered.
