Myocarditis in Children

Abstract

Myocarditis is an inflammatory disease of the myocardium caused by different aetiologies, principally viral infections. The clinical scenario is heterogenous, from asymptomatic/pauci-symptomatic presentation to cardiogenic shock or sudden arrhythmic death. Thus, it is a challenging, sometimes life-threatening disease due to its possible complications, representing an important cause of morbidity and mortality in children. Clinical diagnosis (“clinically suspected myocarditis”) can be obtained by non-invasive investigations, while endomyocardial biopsy still remains the gold standard for diagnosing myocarditis.

Treatment of myocarditis remains largely supportive. Immunosuppressive therapy can be useful in patients with giant cell and active infectious-negative lymphocytic myocarditis. Specific treatment strategies have been proposed (i.e., targeting viral infection or inflammation), but the efficacy of these strategies remain controversial, so far.
