Abstract

The aim of pelvic osteotomies is to improve femoral head coverage and optimize the joint mechanics by increasing the weight-bearing area and by transforming shearing forces into compressive forces to prevent and/or delay degenerative changes. This could be achieved by either reconstructive procedures or augmentation (salvage) procedures. The selection process of the proper osteotomy is the most important step in the management of patients with defective femoral head coverage. Age of the patient, underlying pathology, grade of the disease, presence of osteoarthritis, and congruity between the femoral head and acetabulum should guide the surgeon to choose the optimal osteotomy type. This chapter will shed light on the indications, contraindications, advantages, disadvantages, surgical techniques, clinical results, and complications of different types of pelvic osteotomies.
