Hysteroscopy During Pregnancy

Atlas of Hysteroscopy

Abstract

This chapter allows us to show the evolution of hysteroscopy in pregnancy since the first report of Björn in 1954. Special mention is made to the publication of Oscar Agüero in 1966 where for the first time the title of “a new diagnostic tool” was given to hysteroscopy, in a time when there was no ultrasound allowing diagnosis of placenta previa, hydrops fetalis, fetal death, or many other conditions. Later, for several decades hysteroscopy was only used for intrauterine device removal. Even when contraindicated after the second trimester of pregnancy, over time, other diagnoses and pathological conditions have been included, such as pathologies associated with pregnancy or during pregnancy itself.

In this video an intra-amniotic view is made where the yolk sac is observed attached to the implantation site. There is a thin dark line that suggests necrosis of the umbilical cord by thrombophilia (MPG 54770 kb)

In this case, the IUD inserted in the isthmus is seen; the gestational sac is observed in the fundus; and on the anterior side of the isthmus the IUD removal technique can be seen (MPG 12564 kb)

View of cervical pregnancy in a case of 13 weeks of gestation. The access technique and the section of the umbilical cord for later extraction are appreciated (MPG 120594 kb)

A hysteroscopic view of the cornual pregnancy is made where the coagulation of each of the blood vessels can be seen until they are separated from the uterine horn wall (MPG 2771 kb)

A hysteroscopic view of the cornual pregnancy is made where the coagulation of each of the blood vessels can be seen until they are separated from the uterine horn wall (MPG 5167 kb)

A hysteroscopic view of the cornual pregnancy is made where the coagulation of each of the blood vessels can be seen until they are separated from the uterine horn wall (MPG 6869 kb)

In this video we can see the technique of isthmocele correction prior to the hysteroscopic treatment of ectopic pregnancy in the isthmocele. The intrauterine-cervical heterotopic pregnancy is managed as a cervical pregnancy (MPG 78588 kb)
