Abstract
Arrhythmia after surgical MAZE procedures is often misinterpreted as recurrence of atrial fibrillation. This case demonstrates recurrent mitral annular flutter as a result of incomplete surgical mitral isthmus ablation. Examination of local electro grams provides an instructive diagnostic demonstration of linear ablation breakthrough.
