Abstract

As the trajectory of a patient’s medical course approaches its end stage, assessing a patient’s prognosis through tools such as the FAST score and the “surprise question,” and using communications frameworks such as the “Ask-Tell-Ask” model can assist in engagement of patients and families in goals of care discussions. Home-based medical care providers should be skilled at providing primary palliative care and refer patients to palliative medicine specialists or to hospice care when appropriate.
