Monteggia Fracture-Dislocation: Ulna and Radius Fixation

Abstract

The treatment of patients with Monteggia fractures should begin with a discussion regarding postsurgical expectations and the high risk of complications in order to maximize patient satisfaction. In the operating room, a stepwise approach should be taken in order to turn complex fracture patterns into simpler, more manageable ones. Mini fragment plates can be considered as a reduction tool to help accomplish this. Stabilization of the radius fracture and reduction of the radial head can aid in gauging anatomic length of comminuted ulna fractures. To maintain stability of the radial head, restoration of ulnohumeral joint and ulnar length is required. After surgery, early active range of motion is essential in preventing stiffness.
