Urinary Diversion: The Last Resort of Male Urethral Reconstruction

Abstract

Urinary diversion for the management of recurrent urethral stricture disease should be part of the reconstructive Urologist armamentarium of surgical techniques for this difficult dilemma. This chapter summarizes the reconstructive treatment options and considerations for these difficult to manage urethral strictures, for which urethroplasty or revision urethroplasty have failed or are no longer indicated. Both continent and incontinent urinary diversion options are described, including: Monti and Casale technique, ileoceccocystoplasty, ileovesicostomy, ileal conduit with and without concurrent cystectomy, and bladder neck/urethral closure techniques. Operative outcomes from contemporary series for each of the above techniques are described to guide treatment selection to optimise patient quality of life and decrease urinary tract complications.
