Quality of Life in Long-Term Clinical Forensic Psychiatry

Abstract

A substantial group of mentally disordered offenders (MDOs) does not seem to benefit sufficiently from current forensic psychiatric treatment and might therefore need (life)long forensic psychiatric care. Apart from the serious ethical issues of (life)long deprivation of liberty, it raises the question of what to do with these individuals. Quality of life (QoL) can offer an outcome that serves individual treatment planning, facilitates treatment engagement and is also apt for service evaluation. QoL is a broad concept that seems to intuitively be understood by everyone. In a highly restrictive context such as long-term clinical forensic psychiatry, QoL, however, might have different connotations. For example, autonomy, freedom, sense of control, restriction of movement and constraint of sexual relations in long-term clinical forensic psychiatry are all different than in less restrictive settings or society. QoL assessments should therefore reflect the experiences and perspectives of mentally disordered offenders. Although the number of QoL studies in clinical forensic psychiatry is limited, some relevant aspects for everyday clinical practice have been identified. This chapter will consider aspects of QoL and their implications for everyday clinical forensic psychiatric care. Although QoL outcomes need to be interpreted with caution, long-term clinical forensic psychiatry could benefit from QoL assessment and treatment aimed at QoL enhancement.
