Transitioning from Pediatric to Adult Care in Endocrinology

Abstract

The transition from pediatric to adult care for young adults with chronic endocrine disorders poses unique challenges to the pediatric provider tasked with ensuring continuity in the safety and effectiveness of care. This chapter offers practical recommendations for pediatric endocrinology providers in facilitating transition to adult endocrinology care based on expert consensus guidelines. First, providers must set clear expectations. Next, they must adequately prepare patients for transition by completing readiness assessments, educating patients on self-care, establishing a transition plan in partnership with patients and their families, and creating a summary of medical care. While coordinating transfer to the adult world is a key responsibility for the pediatric provider, it is also important that the pediatric team ensure that patients have established adult care in order to promote care continuity and optimize health outcomes.
