Abstract
Early and appropriate treatment of seizures is important for optimum seizure control and to avoid the consequences of continued seizure activity. With more than 20 antiepileptic drugs (AEDs) now available, choosing the best medication(s) for each patient can be challenging. Appropriate medication selection begins with an understanding of the various mechanisms of action, pharmacokinetics, side effect profile, and treatment of other comorbidities. Nevertheless, despite treatment with proper AEDs, up to 40% of patients will have drug resistant epilepsy (DRE). The importance of DRE must be emphasized as it may lead to developmental delay in children, hinder the development of interpersonal and vocational skills, cause psychological distress, memory problems, and is associated with an increased mortality rate. Early identification of DRE and timely referral to an epilepsy center for surgical evaluation is essential to allow for the greatest chance of improvement. Several options for the treatment of DRE are available and include diet therapy, standard resection, stimulation devices, and other procedures.
