Histoire de la chirurgie de la douleur — traitement chirurgical de la névralgie faciale

Résumé

Les douleurs atroces du « tic douloureux » ont contraint les chirurgiens à s’y intéresser bien avant la naissance de la neurochirurgie. Ces interventions, dans un premier temps, chercheront à interrompre les fibres sensitives incriminées. Au niveau des branches, déjà, par les neurotomies, puis du ganglion — la gassérectomie — et enfin à la racine du trijumeau avec la neurotomie rétrogassérienne par voie temporale ou sous-occipitale. Toujours dans cette logique lésionnelle, la voie percutanée sera également explorée. La neurolyse alcoolique, l’alcoolisation ou la thermocoagulation du ganglion de Gasser chercheront à reproduire les résultats des interventions précédentes sans les risques d’une chirurgie ouverte. Parmi ces techniques percutanées, la compression par ballonnet et la thermocoagulation appartiennent à l’arsenal thérapeutique actuel. Une meilleure compréhension de la physiopathologie aboutira, dans un second temps, à un nouveau geste — conservateur cette fois : la décompression vasculaire microchirurgicale. Plus récente et non invasive, la radiochirurgie visant la racine du nerf trouve aujourd’hui sa place au côté de la décompression vasculaire et des gestes percutanés en cas d’échecs du traitement médicamenteux de la névralgie faciale.
