Abstrait
La majorité des cancers de la vulve, surtout ceux qui surviennent chez les patientes avant la ménopause, sont causés par le HPV. La fréquence de ce cancer est en hausse particulièrement chez les jeunes femmes. C’est une réalité qu’il faut garder en mémoire lors du diagnostic différentiel de lésions irrégulières et/ou érosives de la vulve, et faire une biopsie en cas de doute.
Le traitement des cancers de la vulve est avant tout chirurgical. Des techniques plus conservatrices sont maintenant proposées. La technique du ganglion sentinelle est de plus en plus répandue et permet de réduire le taux de complications de la lymphadénectomie inguinale. La vulvectomie partielle est préférée à la vulvectomie radicale afin de donner des résultats cosmétiques avantageux ; et cela, sans réduire le taux de guérison qui est de plus de 90% lorsque les ganglions inguinaux sont libres de métastases.
La radiothérapie associée ou non à la chimiothérapie joue un rôle important chez les patientes atteintes d’une tumeur localement avancée.
