Malgré des services rendus qui sont indéniables, la cytologie fait l’objet d’attaques régulières en raison d’une sensibilité médiocre. On peut citer en particulier la méta-analyse de Fahey qui nous donne une sensibilité générale de 58 %. L’adjonction du test viral permet d’améliorer cette sensibilité au prix d’un surcoût que les pouvoirs publics ne semblent pas décidés à supporter. Il faut également rappeler que le but d’un dépistage est de sélectionner par un outil idéalement sensible et spécifique une partie de la population susceptible d’être porteuse de la pathologie. Dans le cas présent, le but recherché est la détection des lésions précancéreuses du col utérin à un stade pré-invasif permettant ainsi la prévention du cancer. Trop souvent, on nous présente la cytologie ou la virologie comme un outil de diagnostic. En fait, la cytologie comme tout autre moyen de dépistage ne fait que sélectionner les patientes qui seront à examiner au colposcope à la recherche d’une lésion dont la nature exacte ne sera affirmée que par l’histologie (biopsie ou conisation en cas de biopsie impossible ou aléatoire). Dès lors, il faut reprendre toute la chaîne de l’investigation cervicale dont on sait que certains maillons sont plus faibles et le clinicien doit toujours garder un esprit critique au cours de la démarche diagnostique.
