Outcomes of Robotic Radical and Partial Nephrectomy

Abstract

In the past 20 years, the traditional approach of open partial or radical nephrectomy has been supplanted by minimally invasive techniques incorporating laparoscopic technology with or without the assistance of a robot. When new ­technologies emerge, their efficacy must be proven by comparison to the accepted standard. Laparoscopic approaches for nephrectomy have withstood the test of time and have proven themselves to be equally as effective and arguably superior to the open approach. The emergence of robotically assisted surgery has advanced laparoscopic technology one step further by providing a magnified, three-dimensional (3-D) view with reduced tremor and articulating robotic instruments that offer a greater range of motion compared to traditional laparoscopic instruments. Though these advances may allow for more precise dissection and greater ease of intracorporeal suturing, the translation to improved outcomes for patients requiring renal surgery remains to be demonstrated. Herein, we review the current literature evaluating outcomes of robotic radical and partial nephrectomy, with a specific focus on comparison to conventional laparoscopic techniques. We will also present our techniques of robotic partial nephrectomy, both the transperitoneal and retroperitoneal approaches, based on our previous laparoscopic experience and the growing skill set with the robotic surgical platform.
