29-year-old male with a history of syncope and an abnormality noted on ECG

The left ventricle is minimally elongated in the vertical axis. The septum is noted to be dyskinetic from the basilar level to the apex. The anterior, anterolateral, inferolateral and inferior walls of the left ventricle demonstrate minimally reduced wall motion.

Delayed enhancement imaging demonstrates transmural enhancement of the antero-septum at the basal and midventricular levels, with preservation of the apical septum. In addition, extensive enhancement of the right ventricular free wall is noted. A small focus of possible enhancement is noted in the lateral wall of the left ventricle as seen in the fourth image of the third row.
