Abstract
Optical urethrotomy is a procedure in which a urethral stricture is incised under direct visualization endoscopically. A specialized scope called an urethrotome (Figure 18.1) is used. This instrument has a knife blade that is deployed by the surgeon, and which faces in the upward direction (1). In general, it is indicated for first time strictures as long as they are relatively short in length, and do not appear overly dense by cystoscopy. It can be utilized for recurrent small strictures after urethroplasty as well. More complex urethral strictures can be incised as well with this technique, although the failure rate is generally much higher. Patients who would otherwise not tolerate a more extensive open reconstruction are also candidates.
