The calcium—vitamin D—parathyroid hormone (PTH) system plays a critical role in both health and disease. Despite longstanding acceptance of its importance in maintaining the skeleton, recent and accumulating data have significantly enhanced our understanding of the pathophysiology of cal-ciotropic hormones in the setting of osteoporosis. Herein we review this information and make recommendations based on these studies.
