Abstract
Ruptured abdominal aortic aneurysm (rAAA) is a highly lethal condition with an overall mortality rate of approximately 90%. Patients who survive to present to the hospital require an intervention to reestablish aortic integrity, otherwise hospital mortality is certain. Traditional open surgery, with placement of an interposition graft under general anesthesia, is a durable form of repair. Despite advancements in surgical technique and post-operative critical care, the mortality associated with this form of treatment in the setting of rAAA remains high.
Endovascular aneurysm repair (EVAR) has emerged as an alternative to open surgery for the elective treatment of AAAs with short- and long-term advantages in aneurysm-related mortality. This form of therapy is more commonly being applied to the treatment of rAAAs. The application of EVAR to rAAAs has several inherent advantages over open surgical reconstruction, which may translate into improved patient outcomes. Several challenges inherent to EVAR must be addressed in order to successfully apply this form of treatment to rAAAs in the acute setting. Early reported outcomes of utilizing EVAR for rAAAs have demonstrated the technical feasibility of this approach, and several studies have suggested a mortality advantage over conventional open surgery.
