Transitional Cell Carcinoma of the Renal Pelvis: Management

Abstract

Upper urinary tract transitional cell carcinoma (UUTTCC) accounts for approximately 5% of all urothelial tumors. Surgical therapy remains the standard of care for patients with localized disease based on tumor stage and grade. Surgical options for treatment range from minimally invasive procedures, such as ureteroscopy, and laparoscopic nephroureterectomy to open radical nephroureterectomy. Surveillance with cystoscopy and cytology following surgical management is essential as currently there are no variables that consistently predict which patients are destined to develop intravesical and/or contralateral upper tract recurrences. Extrapolating from traditional bladder TCC, both regional lymphadenectomy as well as neoadjuvant and adjuvant chemotherapy are likely to be beneficial for this cohort of patients. Management of patients with advanced UUTTCC should follow current guidelines utilized for patients with bladder TCC.
