Abstract

Our increasingly sophisticated understanding regarding the epidemiology, pathophysi-ology, and treatment of cardiovascular disease (CVD) has only served to underscore the continuing need for primary and secondary prevention efforts. Many of these insights have been incorporated into updated practice guidelines. At the same time, however, there is little evidence that the “treatment gap”—the troubling disparity between such endorsed guidelines and actual clinical practice–has narrowed. This is particularly true in ambulatory settings, and in clinical encounters in which a solo physician is the source of preventive services. The resulting need to translate research into practice for more effec-tive CVD prevention is undiminished, as is the opportunity to develop innovative models for such care.
