Abstract

Erectile dysfunction is a significant quality-of-life issue that affects both genders. One in four men have difficulty achieving an erection by age 55. The incidence of this problem is 50% in men by age 65, and the majority of men over age 70 cannot achieve an erection suitable for intercourse (1). The penis has two elastic chambers that are central to erection. They are covered by an elastic membrane, the tunica albunginea, and contain cul-de-sacs surrounded by smooth muscle. Various stimuli influence this smooth muscle to relax. As this occurs, blood flows into the cul-de-sacs, expanding the tunica albuginea to its limits of stretch and compressing venules that run underneath it, thus preventing blood from leaking out of the chambers. This gives increased size and rigidity to the penis. Other stimuli cause this smooth muscle to contract, relaxing the erection and maintaining the penis flaccid.
