Salvage Surgery After Chemoradiation Therapy

Abstract

The past two decades have seen significant advances in the efficacy of combined chemotherapy and radiation in the management of advanced stage squamous cell carcinoma (SCC) of the head and neck. This is especially true for SCC of the oropharynx, hypopharynx, and larynx. Primary surgery for advanced T-stage tumors, despite sophisticated reconstruction of the surgical defect, may result in a significant impact on speech, swallowing, and respiration. Organ preservation protocols using definitive radiation with or without chemotherapy may provide an effective primary treatment while avoiding the functional and cosmetic morbidity associated with surgery.
