Abstract
The predisposition to infection experienced by patients with diminished or absent splenic function is well-documented (Brozovic, 1994). However, the susceptibility to, and severity of, an infection are dependent on many other variables. The virulence of the infecting microorganism is of critical importance. The degree of resistance mustered by the asplenic or hyposplenic patient is determined by the age of the patient and age at the time of splenectomy (or significant splenic dysfunction), duration of asplenism, underlying diseases that may cause further immunosuppression, and therapeutic interventions, including chemotherapy and radiation. A discussion about the relationship of the spleen to infection must include not only attention to splenic dysfunction, both anatomical and functional, but must also incorporate the relationship between the microorganism and its host, the patient.
