Postoperative Portal, Mesenteric, and Splenic Vein Thrombosis

Abstract

Portal, mesenteric, and splenic vein thrombosis (PMS-VT) is a major postoperative complication following HPB procedures, caused by a combination of systemic and loco-regional prothombogenic factors. The clinical symptoms are mostly unspecific and variable, which makes an accurate diagnosis difficult. Patients with documented PMS-VT need to be initially treated with anticoagulation to prevent propagation of the thrombus. Other treatment modalities have been investigated, including percutaneous thrombolysis, balloon angioplasty and/or stent placement, and surgical thrombectomy. Treatment of PMS-VT should be determined by the individual clinical situation of the patient, the pathophysiology involved, and the available expertise.
