Abstract

Lung injury can result from a wide range of acute or chronic pulmonary and extrapulmonary pathologies (Figure 3.1). Acute lung injury in adults can produce a spectrum of clinical presentation, at the extreme end of which is the syndrome of acute respiratory distress (ARDS). Since its original description as a disease entity [I], ARDS has been the subject of intensive research, but the limitations of this abbreviation in describing such a diverse clinical condition and group of patients have become increasingly apparent. As the epidemiology, aetiology, natural history and pathology of acute lung injury have been researched, it has become harder to define what condition is defined by the term ‘ARDS’ [2,3]. It has also become apparent that studies utilizing strict clinical definitions of the condition exclude subgroups of patients.
