Abstract

Pulmonary injury in the syndrome of acute respiratory distress in adults (ARDS) results in disruption of the alveolar-capillary membrane by mechanisms acting directly via the airway or indirectly via the bloodstream [1]. Indeed, within hours of injury there is epithelial and endothelial cell damage and denudation, with exposure and disruption of the epithelial and endothelial basement membranes. Microthrombi form in damaged pulmonary capillaries [2], and breakdown of the alveolar-capillary membrane occurs allowing protein-rich interstitial fluid to flood the alveolus and severely impair gas exchange [3]. Hyaline membranes form [1], and inflammatory cells, initially predominantly activated neutrophils, accumulate in the lung, particularly in the alveolar space (Chapter 4).
