Abstract

The editors of this text on the syndrome of acute respiratory distress in adults (ARDS) have insisted, quite rightly, that the first chapter should begin with some introductory comments and a definition of the subject that the rest of the book is about. The easy part of the assignment is the introduction, which will take the form of an historical overview and a brief examination and evaluation of some of the pathogenetic concepts that have evolved as medical scientists have tried to understand the origins of this fascinating but elusive disorder. Defining it, however, is not as easy as it sounds, because there is no uniformly accepted definition of the syndrome, and the various definitions that have been used by workers in the field since the term was introduced in 1971 have differed considerably.
