Thyroid Regulation During Pregnancy

Abstract

For a few years, there is renewed interest in the regulation of maternal thyroid function during pregnancy. Our laboratory has recently been involved in the investigation of several aspects of this question, dealing with the adjustment of maternal thyroid function during gestation, both in healthy subjects1 and in women with underlying thyroidal abnormalities2, the goitrogenic stimulus of pregnancy in relation to iodine restriction3,4 and the repercussions in newborns of moderate iodine deficiency in the mothers5. Basically, the thyroidal challenge can be viewed as a mathematical fraction with hormone requirements in the numerator and the availability of iodine for the maternal thyroid in the denominator. The thyroidal challenge therefore arises because hormone requirements are increased during gestation, while the availability of iodine is reduced.
