Abstract

Since the 1960s, alkylating agents have been used to treat immune disorders. These agents have also been commonly used in bone marrow transplantation in order to condition the graft recipient, thereby reducing the risk of graft rejection. Cyclophosphamide has been included less frequently in immunosuppressive protocols for other transplant procedures, including renal transplantation. Their most controversial role, however, is in the treatment of autoimmune and chronic inflammatory diseases, particularly those resistant to conventional treatment. This chapter considers the place of alkylating agents in the management of immune disorders. Their contribution to transplantation is described in the appropriate literature. Only chlorambucil and cyclophosphamide are considered in detail because, for practical purposes, these are the only alkylating agents regularly used to treat immune disorders.
