Abstract

The relationship between diabetes mellitus (DM) and hypertension has been recognized since the 1920’s.1–3 Diabetics are at increased risk for atherosclerotic complications and hypertension increases this risk. It is now becoming evident that complications associated with diabetes, including nephropathy and retinopathy, are related not only to the duration of diabetes and degree of diabetic control but also to the presence, duration and severity of hypertension. Furthermore, the relationship of hypertension to diabetes differs somewhat between type I DM (insulin-dependent [IDDM], juvenile onset, ketosis-prone) and type II DM (non-insulin-dependent [NIDDM], adult onset, nonketotic) and could modulate the prognosis of diabetes in the respective diabetic groups. The case summaries of three diabetic patients discussed below illustrate important differences and similarities in the etiology and natural history of hypertension in diabetic patients.
