Abstract

Any chain is as strong as its weakest link This is a Dutch saying, and readily applicable to the blood transfusion chain. Not strictly Dutch though is our stubborn unwillingness to pay blood donors for their donations. Why? Is it our well known strive for cost-containment? Not sure. Voluntary non-remunerated donation is the basis for a safe blood transfusion policy. This is the single most powerful achievement of transfusion safety since the nineteen-seventies. However as with any powerful kingdom, its sovereignty is readily down-played. Not because of new medical or scientific insights, but more down to earth by financial interests. The king is not dead yet however.
