Blood transfusion is one of the routes for transmission of disease. Prevention of such transmission depends primarily on structure and organisation of the blood supply from vein to vein, the human resources involved and their education and training as well as their authority, responsibility and accountability. Above all it depends on the implementation and functioning of a quality system and quality management. Key elements are leadership at any level in the organisation, human resources and quality awareness.
