Calcium is present in protein-bound, complexed and free forms in plasma. Serum values vary according to patient age, sex and albumin concentrations. Serum phosphate levels vary inversely with calcium to maintain a constant product of their concentrations between 30 and 40. A 1 gmJdl change in serum albumin concentration is associated with a 0.8 mg/dl change in total serum calcium. Ionized calcium is the physiologically relevant portion and its concentration is dependent on the pH of the blood. If serum protein concentrations and blood pH are normal, hypercalcemia in adults is defined as serum calcium exceeding 10.5 mg/dl.
