Abstract

In the 1983 edition of Clinical Oncology,1 the authors note, “The outlook for the patient with any alimentary tract cancer is not very favorable...) and these cancers are mainly of surgical interest.” Although cancer of the gastrointestinal tract remains of surgical interest, strides in the understanding of natural history, early diagnosis/ treatment, and pathogenesis have brought cancers of the gastrointestinal tract into a new therapeutic realm. This chapter reviews the changes that have occurred in past 10 years in the diagnosis and treatment of these cancers, with particular emphasis on the current approach to treatment of cancer of the colon and rectum in the older person. Chapter 18 discusses the major impact on treatment that has come from advances in early diagnosis.
