Experience with Anesthesia-Free Shock Wave Lithotripsy Using the Unmodified Dornier HM3 Lithotripter

An unmodified Dornier HM3 lithotripter was used for anesthesia-free extracorporeal shock wave lithotripsy (ESWL*) treatments of urolithiasis on 1,433 occasions. The requirement for anesthesia in this series was only 1.6%. Premedication with pethidine and diazepam was followed by local application of an anesthetic cream (EMLA) Small as well as large stones usually were treated successfully with a generator voltage between 14 kV and 16 kV. For stones with a diameter smaller than 20 mm, the retreatment rate was the same with the anesthesia-free approach as with the original anesthesia-requiring procedure. Larger stones had a higher need for repeated sessions, but four weeks after treatment, the disintegration was considered similar with the two methods. These results show that most renal and ureteral stones can be successfully treated with an unmodified Dornier HM3 lithotripter.
