Abstract
Biclonal gammopathy is defined as the syndrome in which the serum of a given individual contains abnormally large amounts of two distinct, homogeneous populations of immunoglobulins. Other terms used to describe this state are biclonal myeloma, double myeloma, and oligoclonal gammopathy. Biclonal gammopathy is associated not only with multiple myeloma; it has been observed in individuals with other types of malignant and nonmalignant diseases as well as in individuals with apparently benign conditions. In contrast, monoclonal gammopathy is an increase in a serum immunoglobulin that has the elctrophoretic mobility of a monoclonal protein and is often a manifestation of the neo-plastic process associated with multiple myeloma or Waldenström’s macroglobulinemia. This immunoglobulin is usually a chemically homogeneous product secreted by a clone of neoplastic lymphocytes or plasma cells. The protein is monoclonal because it comprises one type of heavy chain (i.e., α,γ, μ, δ, or ∈) and one type of light chain (k or λ).
