Electrocardiographic Aspects of the Preexcitation Syndromes

The preexcitation syndromes are characterized by the existence of abnormal propagation of depolarization from the heart’s atrium to its ventricles which results in early excitation of the ventricles and can result in deformation of the QRS complex. This unusual excitatory sequence coined “anomalous atrioventricular excitation” [1] or “preexcitation” [2] interested early electrocardiographers and was reported in isolated cases or small series by Cohn and Fraser in 1913 [3], Wilson in 1915 [4], Wedd in 1924 [5], Bain and Hamilton in 1926 [6], Bach in 1929 [7], and Hamburger in 1929 [8]. In 1930, Wolff, Parkinson, and White described in 11 patients a syndrome with the ECG features of “bundle branch block with short PR interval in healthy young people prone to paroxysmal tachycardia” [9]. Subsequently, the WolffParkinson-White (WPW) syndrome eponym describing the most frequently observed of the preexcitation syndromes, has become established in the lexicon of cardiology.
