Abstract

The clinician’s task of diagnosing cocaine abuse is significantly complicated by the common tenacity of drug users in denying their drug consumption. A drug user’s denial is fueled in part by the stigma society attaches to drug use and by the fact that admission of drug use threatens the continuation of the drug-using state. As a result, patient histories are a notoriously unreliable means of identifying drug use. Figure 4.1, derived from an antenatal and postpartum study of inner-city black mothers, illustrates the underreporting of contemporaneous drug use. In this study, drug use was assessed by interviews during pregnancy and 13 months after giving birth. The findings of this study that drug use during pregnancy was significantly underreported confirm other studies of different populations.1
