Free Fatty Acids, Catecholamines and Arrhythmias in Man

Abstract

The role of free fatty acids (FFA) in the pathogenesis of malignant arrhythmias associated with myocardial ischemia or infarction remains controversial. Such an association first was suggested by Kurien and Oliver in 1970 (1) in their “FFA hypothesis” on the basis of clinical observations in patients following acute myocardial infarction. It was proposed in their hypothesis that increased substrate availability of FFA to the ischemic myocardium either alone or acting synergistically with catecholamines might exert adverse electrophysiological effects. Subsequent clinical and experimental evidence, however, has been confusing and contradictory and the concept remains neither fully confirmed nor refuted.
