Abstract
The sequence of events from asymptomatic hyperuricaemia to acute gout, intercritÃcal gout and finally chronic tophaceous gout is well established, one fifth of patients who suffer acute gout eventually developing tophaceous deposits if untreated with hypouricaemic drugs. Deposition of tophi in the absence of acute gouty arthritis is extremely rare: the cases described here invite speculation on the mechanism of inhibitory factors involved in acute gout.
